Dr. Supriya Rao, a gastroenterologist, points to four modifiable drivers behind the surge:

  • Metabolic dysfunction–associated steatotic liver disease (MASLD) (formerly NAFLD) fueled by obesity

  • Poor diet—especially ultra-processed foods and added sugar

What we’re seeing now is a perfect storm: rising metabolic illness, liver fat accumulation, and missed opportunity in screening and prevention.

But thanks to early detection and simple steps, this trajectory isn’t inevitable

Here’s what your doctor can—and should—do:

  • If you’re a higher-risk patient, you should be screened using non-invasive liver imaging (but keep in mind, fatty liver often shows no symptoms until it’s advanced)
  • Ask about alcohol and metabolic health—even during routine cancer follow-ups
  • Collaborate across specialties to catch early MASLD and prevent progression
  • Counsel you on lifestyle shifts—weight loss, reducing added sugar, limiting alcohol

If you’re looking for a research-supported supplement that may support liver health, milk thistle (the extract silymarin) has been studied in people with MASLD/NAFLD.

A meta‑analysis of 26 randomized trials involving over 2,300 patients found that silymarin helped:

  • Reduce levels of liver enzymes ALT and AST
  • Improve liver fat index and steatosis grade
  • Lower LDL cholesterol and triglycerides, improve glucose and insulin resistance
  • Reduce inflammation and oxidative stress

One smaller trial in obese patients showed improved ultrasound grading and liver enzymes after just eight weeks.

While more studies are needed, silymarin appears to act as an anti-inflammatory, antioxidant, and metabolic supporter in fatty liver disease.

Here’s what you can do today:

Start by getting assessed for fatty liver—especially if you have obesity, diabetes, or drink regularly.

Be sure to adopt liver-friendly habits. That means choosing whole foods, cutting sugar, moving regularly, and managing weight.

And last, but not least, consider milk thistle, but only after consulting your doctor—especially if you have liver disease or take medications (like blood thinners).
