You’ve heard us talk about GLP-1 medications before.

Back in May 2024, we were one of the first to report on the unexpected potential of these popular diabetes and weight loss drugs—like liraglutide and semaglutide—to slow or prevent Alzheimer’s disease.

And while we’ve never shied away from covering the unwanted effects of GLP-1s—from gut issues to lean muscle loss—we’re also committed to giving you the full picture.

Because when it comes to something as devastating as Alzheimer’s, even a small benefit could be worth the risk.

Now, new research suggests that GLP-1s may actually outperform the long-standing diabetes drug metformin when it comes to protecting the brain…

In a head-to-head study published in BMJ Open Diabetes Research & Care, researchers compared dementia risk in over 87,000 patients with type 2 diabetes. Some were prescribed metformin—the decades-old first-line therapy—while others took GLP-1 receptor agonists.

The results?

Participants taking GLP-1s had a 10% lower risk of developing dementia overall. Even more striking, their incidence rate of Alzheimer’s was nearly half that of those on metformin.

According to lead researcher Dr. Szu-Yuan Wu, GLP-1s appear to lower brain inflammation, improve blood sugar metabolism in the brain, and protect neurons against the very damage that drives Alzheimer’s disease.

And unlike some earlier studies on cognitive protection, this one was based on real-world data—not just lab models or short-term trials.

Experts point out this doesn’t mean everyone should swap metformin for a GLP-1. The medications work differently, have different side effect profiles, and aren’t interchangeable.

But it does suggest that, for patients already at high risk of cognitive decline—especially those with type 2 diabetes—GLP-1s may offer added protection that’s worth discussing with your doctor.

And that’s the key takeaway here: informed, individualized care.

We’re not saying GLP-1s are miracle drugs. We’ve written before about the serious concerns—nutrient loss, muscle wasting, the unknown long-term effects. But we also know this: When your brain is on the line, you deserve every piece of data available to weigh the risks and rewards.
