Researchers at Duke University ran a 12-month pilot study with patients diagnosed with NAFLD. Participants took betaine daily.

By the end of the trial, liver enzyme levels told the story…

ALT—a marker of liver damage—dropped 39%

AST—another liver enzyme that signals inflammation—fell 38%

Those aren’t minor improvements. Those are the kinds of reductions that indicate actual REVERSAL of liver damage.

But researchers wanted to know: does betaine actually reduce the fat itself, or just the inflammation?

A 2025 study tested three different doses—8 grams, 4 grams, and 2 grams daily—over 12 to 24 weeks.

All three doses significantly reduced liver fat accumulation starting at 12 weeks. All three brought liver enzymes back into normal range.

Even the lowest dose worked!

A 2024 study identified the exact pathway: betaine activates something called the BHMT/FTO pathway, which directly improves how your liver handles insulin and processes fat.

When insulin signaling improves, your liver stops hoarding fat and starts burning it.

Betaine supplements are widely available as betaine anhydrous or trimethylglycine (TMG).

The studies used doses ranging from 1.5 to 3 grams daily, taken with meals. Some trials went as high as 6-8 grams with no adverse effects.

Most people start at 1.5-2 grams daily and increase if needed.
