You’re enjoying your morning coffee when a thunderous headache strikes out of nowhere. It’s not like your usual tension headache—and something about it feels different.

Should you wait it out? Or should you call for help?

For seniors, the answer might be more urgent than you think.

New research and guidance suggest that some headaches—especially when they come on suddenly, change in intensity, or are accompanied by other symptoms—can be red flags for serious, even life-threatening conditions.

Most headaches are harmless, but some require immediate attention—especially in people over 60.

According to leading neurologists, the following symptoms should never be ignored:

  • A sudden, severe “thunderclap” headache
  • Head pain that begins after the age of 50
  • A headache that worsens when coughing, sneezing, or lying down
  • Pain that always hits in the same spot
  • Headaches with fever, night sweats, or unexplained weight loss
  • Accompanying vision changes, numbness, or slurred speech
  • A headache paired with confusion or facial drooping

If you—or a loved one—experience any of these, seek medical help immediately. Call 911 rather than trying to drive yourself to the ER.

Here are just a few of the medical emergencies that can present with headache as an early warning sign:

  • Stroke: Often accompanied by drooping on one side of the face, slurred speech, and weakness in the arms or legs.
  • Brain Tumor: Persistent headaches, seizures, or changes in personality or vision.
  • Heatstroke: Especially dangerous for older adults in warm climates. A headache paired with confusion, nausea, or flushed skin could be an emergency.
  • Carbon Monoxide Poisoning: This silent killer may show up as a headache, nausea, dizziness, and fatigue—especially in winter months.
  • Carotid Artery Dissection: A tear in a major artery supplying the brain, often showing up as one-sided headache with neck pain or vision issues.

Even something as seemingly minor as a concussion from a small fall can result in a dangerous bleed in the brain for older adults. Headache may be the only symptom—so if you’ve hit your head recently and notice a new headache, it’s time to get checked.

A sudden, pounding headache could also be your body’s warning that your blood pressure has spiked into dangerous territory.

A hypertensive crisis—when readings jump above 180/120—can result in stroke, heart attack, or kidney damage. Shortness of breath, nosebleeds, or intense anxiety can also accompany this kind of event.

If you’re at risk, make sure to regularly monitor your blood pressure—and talk with your doctor about strategies to keep it under control.

While some headaches require immediate medical attention, there’s a lot you can do to prevent more routine ones—especially if you suffer from frequent tension or migraine headaches.

Here are a few senior-friendly, natural strategies to try:

  • Stay hydrated. Dehydration is one of the most overlooked causes of headaches.
  • Limit processed foods. Nitrates and MSG (often found in packaged foods and lunch meats) are known migraine triggers.
  • Manage stress. Regular meditation, deep breathing, or even a short walk can go a long way.
  • Consider magnesium. This mineral helps regulate nerve function and has been shown to help prevent migraines in some people.
  • Watch the weather. Changes in barometric pressure can trigger headaches for some—especially in spring and fall. Tracking patterns can help you stay ahead.

And if your headaches are frequent and unpredictable, it might be worth keeping a journal. Write down what you ate, how you slept, what the weather was like, and how the headache felt. This can help you and your doctor spot patterns—and rule out anything serious.
